CCS-California Children's Services

PROGRAM DESCRIPTION

The CCS program provides diagnostic and treatment services, medical case management, and physical and occupational therapy services to children under age 21 with CCS-eligible medical conditions. Examples of CCS-eligible conditions include, but are not limited to, chronic medical conditions such as cystic fibrosis, hemophilia, cerebral palsy, heart disease, cancer, traumatic injuries, and infectious diseases producing major sequelae. CCS also provides medical therapy services that are delivered at public schools.

Health Plan of San Mateo is the county agency that provides and manages CCS services in San Mateo County.

The CCS Pilot is a partnership between San Mateo County CCS and the Health Plan of San Mateo (HPSM). The goal of the CCS Pilot is to improve care and services for CCS kids by:

  • Coordinating care between specialists and primary care doctors; and
  • Coordinating referrals and authorizations between CCS and HPSM

In the CCS Pilot, a dedicated case manager oversees a child's total care. This includes coordinating social and mental health services for caregivers, in addition to a child's medical services.

In April, 2013, around 1,400 kids from the CCS Program in San Mateo County were enrolled in the CCS Pilot. This is more than half of the total number of kids in the County’s CCS Program
